Claim Watcher® plans have no network restrictions — we help you see the providers you choose for your care.
You can visit any provider or facility at no additional cost, as long as they agree to accept the plan. More than 98% of providers accept the plan when it’s introduced in advance.
Simply submit the Provider Assistance form. We’ll reach out to confirm acceptance and coordinate directly with your provider to help ensure a smooth experience.
HOW IT WORKS

Need care soon?
If you already have an appointment scheduled within the next 3 business days or need to see a provider quickly, please do not use this form.
Instead, call our Concierge member Services team at the number on your ID card or (844) 307-3788 so we can assist you right away.

Submit a request
Before scheduling an appointment, simply complete the Provider Assistance form.
Our team will reach out to your providers to introduce the plan, explain how it works, and answer any questions so you can feel confident before your appointment.
3.
What to expect next
Once we receive your request, our team will contact the provider. We’ll follow up with you within 3–5 business days to share the outcome and next steps.
On the rare occasion a provider is not willing to work with the plan, our team will provide alternative providers for you to choose from.
